Wiring Diagram for 5 pin potentiometer?

I am currently trying to wire up a 5 pin potentiometer and I cant get it to work. I've followed 2 different diagram and both of them didn't work for me. Does anyone have a diagram that has worked for them?

you could have dud potentiometers....

but personally i would recommend not to follow a diagram for the pinouts (only for wiring schematic), but test the pinouts yourself....
