How do you measure: where do you touch your ground port from the measurement device to the display PCB? Because 4V is a bit strange...

Please show a picture of your wiring performed. It is important to have a GND connection from this button PCB to the Arduino. What you measure on the chip output could be some rest of AC ripple. Try to measure on the big capacitor on the picture bottom. There you should get something closure to 5V. You can inject your 5V as well on this cap. Take care: if you measure something close to 3.3V DO NOT inject 5V there!

I recognized the same issue on my builds and have no clue up to now hot to solve it. Since this I just deinstalled autosave in GBA.
I am looking forward to get this solved by this treat!
I roughly read something that this issue could be linked to the rom.

Depending on your HDMI output device. In my case the Kedei screen puts enought analog output signal on its internal headphone jacknto run a speaker. Not really noisy but enough to listen.

First of all: I am interested in a solution as well!
Just for the view of the menue: maybe it helps to reboot the gameboy into HDMI only ( as available since the last update from KITE) inside of retropie and view the game settings on an external HDMI screen.

The easiest builds are without really programming. Just follow guides and copy files on the right places as shown in the guides. For example if you follow gpio guides. If you feel more confident you can do the button controls with a arduino microcontroller. There again: just copy the already prepare...

The additional micro USB port is used often to extend the connection to a better place (as you already wrote).
The internal micro usb port and the later onsoldered external one are just wired in parallel to each other.
