When they're first made (The PCBs) some of the exposed contacts have a slight coating on them which usually gets better and wears off with time. As Konnor said, repeated use makes it get better. If it doesn't, it can sometimes be a poor soldering joint on the Pi to the button that's not working.

I'm currently repairing this, I believe the diagnosis is that the chip is faulty but I'm in the process of fully checking the board.

Unfortunately that's the nature of the beast. Those wheels are of a set value and they don't make them in other values. I investigated adding other inline resistors to give the full wheel volume however it made the audio muffled and less crisp. It doesn't solve it but hopefully that explains why it ...

It's turned off by default already but not in the typical way so disabling this option overwrites that. There is a new image coming that I've hopefully eliminated it from just making a black screen. I'm in testing currently so I'll give this a test.
