Ordered, arrived, and installed and these buttons look and feel great!
I did have a small alignment issue with one button hole like a previous poster but was able to mostly correct it. I suggest going very slowly and hand turning your bit. More important than alignment with the caps and the button membranes (there is a bit of wiggle room here) is ensuring that your holes are centered/aligned with each other so that they look as good as Hoolyhoo's OP image.

I have a question that is slightly off topic. With the fan mount that you designed, have you tried it with Kite's CS and if so, where does it sit above the Pi?
I am asking as I am looking at grabbing a game that doesn't work and doing something similar with the cartridge.

The inserts for these were not pictured, but do they look similar to these?
The image was taken from the "Universal Button L1, L2, R1, R2 Back Button PCB" sale thread. I'm guessing they would somewhat be the same albeit a little smaller maybe?

What about guide for drill holes in right place?

The pcb has holes that are centered where buttons go so built in drill guide. Just mark before soldering the buttons to the pcb.
