I wasn't doing pwm audio though. I had a cable hooked into a breadboard from my phone to the amplifier. There was a bit of noise that did go away when the power was cut, but other than that, no audio.

Other than that bit of noise, no. I do have another stereo amp that I know works. Trying to find it to test my set up!

I'm not getting a result from the other amplifier either. It might be the setup of the headphone jack that's faulty. I did test the output of the known working amp and I was getting a signal, just no audio.
I was getting a signal out of yours too (though it it had noise where the other didn't--could be because I didn't know where to hook up the input audio cable's ground (L-/R-) on yours. I have a USB DAC, so this will probably become an issue for me), just nothing through the audio jack.

Ebay has them as well as Aliexpress or Alibaba.com.
You can use a usb sound card just take it's output to the Volume Wheel pot 1 and pot 2 then solder a wire from pot 3& 4 on volume wheel to pot 3 & 4 on my board.

How would we wire L- and R- when doing usb stereo? I get that L+ and R+ go to pot pins 1 and 2, but not where L- and R- would go.
